Amanda Troxler has been practicing family formation law since 2013. She represents hundreds of Intended Parents, Donors, and Surrogates a year. Additionally, Amanda has personal experience in third party assisted reproduction, having been an egg donor.

In today’s show we are going to explain what family formation law is. As a fertility patient, it’s important that you understand some of the basic legal pieces that you might want to have in place for your journey, whether that be for IVF, or if you’re going to be using donor eggs, donor sperm, or using a gestational carrier. I very much love Amanda’s thoughtful and caring approach to making family formation law approachable. As she says, “healthy families are formed through honest communication and centering the needs of the future child."

Amanda received her J.D. from UCLA law on a Dean’s Merit Scholarship in 2012. She received her Bachelor of Arts degree from Northwestern University in 2008. Amanda is a member of the Academy of California Adoption-ART Lawyers (“ACAL”), Society for Ethics in Egg Donation and Surrogacy (“SEEDS”), and American Society of Reproductive Medicine (“ASRM”).

Thank you for joining me, Amanda!

  • PCOS is one of the most common and most misunderstood conditions that we treat as reproductive endocrinologists, and this week I'm delighted to be joined on the podcast by Dr. Lucky Sekhon of RMA of New York to talk all about it.

PCOS stands for polycystic ovary syndrome, and that name can lead to some confusion. A lot of people think that they probably have cysts in their ovaries. The truth is that all women have multiple cysts (which are really small follicles that house eggs) in their ovaries. Those with PCOS have follicles that look like what we call a "string of pearls" on the periphery of their ovaries, and often have irregular (or no) cycles.

This is the most common question I get from my patients. I get it. By the time you come to see me you’ve been preparing for pregnancy for a year, three, sometimes longer. • Read the full show notes: https://www.draimee.org/dor-decreased-not-depleted-ovarian-reserve

There are a lot misunderstandings around what the term DOR means. Often times when people first hear about DOR, they believe that that the "D" of DOR means "depleted" ovarian reserve. This is not true! To set the record straight, DOR means "decreased" ovarian reserve.
The other thing that happens when the word "decreased" gets mixed up for "depleted," is that many people next assume that getting a DOR diagnosis means that they will need to use donor eggs in order to get pregnant. This is also not true! With DOR, you still have eggs, you just don't have as many eggs as you once did.

In this episode, I'll be sharing more about DOR. At some point, every one with a uterus will go through it. I'll talk about when egg reserves tend to decline, what tests you can take to determine your egg reserve levels, what you can do to keep your eggs healthy, and when you might consider using donor eggs.

Regardless of who you see for fertility care, you deserve the best—personalized treatment. I realize that not everyone can come to see me, and I won’t be around forever.

This information is a valuable resource as you begin your fertility journey, whoever it is with and whenever it takes place.

The inspiration behind this topic? Well—it started with some snooping. Several times each day, I hear my receptionist answer the phone and new patients on the other end asking what they can expect at a new patient consult. The mystery is about to be revealed!

  • Dr. Meivys Garcia is joining me on the podcast today! She is an amazing Canadian fertility doctor, and she's guiding us through her approach to fertility care when a frozen embryo transfer doesn’t work. She will describe every step, one at a time. We will also get personal. Knowing the heartbreak of having failed embryo transfers during her own fertility journey, Dr. Garcia knows all too well what it is like to be a fertility patient.

She’s going to share her journey and share her wisdom based on her experience as a patient and as a fertility expert.

I’m going to be honest and open about this; one of the hardest parts of walking the fertility journey with patients is when we have loss. Whether that be the loss of a pregnancy or the loss of a frozen embryo, it’s heart wrenching for me, and for the patient.

Today’s podcast episode will be about holding space for loss, and how to move on when you’ve had difficult news. It’s about what you can learn from the unfortunate events. It’s also about the hope and courage it takes to be a fertility patient, and the admiration that Dr. Garcia and I have for every patient we’ve worked with.

I’m beyond delighted to have Alice D. Domar, Ph.D back on the podcast to talk about how to support fertility with mind/body strategies and techniques. She is currently the Chief Compassion Officer at Inception, an associate professor in obstetrics, gynecology, and reproductive biology, part-time, at Harvard Medical School, and a senior staff psychologist at Beth Israel Deaconess Medical Center. Dr. Domar established the first ever Mind/Body Center for Women’s Health ~ as well as the very first Mind/Body Program for Fertility. She now teaches people strategies to reduce anxiety, depression, and distress through relaxation, mindfulness, cognitive restructuring, and yoga. Research continues to show that mind/body infertility programs not only improves psychological symptoms, but also increases pregnancy rates. We will be talking about all of this, along with her redesigned mobile app FertiCalmPro (which contains more than 500 solutions to the events and situations most likely to cause distress for anyone experiencing infertility), and her new program with Best Shot. This discussion about the mind/body connection is so very important, because the mental and emotional strain on fertility patients is something that doesn’t get enough attention. • Read the full show notes: https://www.draimee.org/becoming-a-changemaker-on-your-fertility-journey-with-guest-alex-budak Alex Budak is an author and faculty member at Berkeley Haas School of Business. He recently wrote a book called "Becoming a Changemaker." If there is anything I know about fertility patients it's that you all know a lot about being on a journey that often involves embracing change. You become more resilient, and you follow a "growth" mindset. You cling to hope and you let it lead the way forward. It's no surprise that I would have a social entrepreneur and changemaker on the show. You have a lot in common with what Alex teaches and writes about: "Developing a changemaker mindset often requires holding two seemingly contradictory traits simultaneously: Having a strong personal vision yet being open to collaborating with others. Employing drive and determination to push through barrier after barrier yet knowing when to step back and recharge." Today we're talking about how to become a Changemaker on your fertility journey. Here are three traits he believes a changemaker has: 1. There's always another way
    2. Existing at the edges
    3. Learned Hopefulness / Learned Optimism

So much is misunderstood about PCOS! Women are often told, “oh you don’t need to worry about it until you’re ready to have babies.” That could not be further from the truth. Today on the podcast, I'm talking about 5 novel treatments for PCOS.

  • Today on the Egg Whisperer Show podcast, Dr. Geoffrey Sher joins me again to talk about his approach to care when it comes to IVF. We are discussing how he individualizes IVF protocols to optimize egg and embryo quality, giving his patients the best chance of IVF pregnancy. As he shares, there is no "one size fits all" approach to choosing the best protocol for IVF. It's no surprise that I agree with him. Protocols need to be customized, and are dependent on the woman's age and her ovarian reserve. Tune in as we talk about how to pick a protocol, the role of the trigger shot, the importance of hormones like estrogen and testosterone, and EZ-IVF versus mini IVF.

Jenny has an amazing story. On her fertility journey, she has gone through IUI, IVF, had ovarian cancer, and later she and her husband used donor eggs to create embryos carried by a surrogate to realize their dream of growing their family.

In our interview, Jenny is so transparent about everything she went through. She is open about the challenges she faced in fertility treatment, she explains how IVF saved her life, and she opens up about being nervous about bonding with her child. She talks about how her husband approached the topic of using a donor egg, being a donor conceived person himself.

There is so much goodness and hope in our conversation. I know that anyone who has been in fertility treatment, or who has had cancer impact their fertility will be inspired by what she has to share. Thank you for being on the show, Jenny!

  • Read the full show notes: https://www.draimee.org/preparing-for-embryo-transfer On today’s podcast episode, I’m talking about preparing for embryo transfer. I find that it’s helpful to think about embryo transfer the same way you might think about having a very special guest over for dinner.

What are you going to do if you have a very special person over? You’re going to make sure everything is absolutely perfect. When it comes to preparing for an embryo transfer, you want to take the same approach. Your metaphorical home in the house analogy is your uterus.

The devastation of doing more tests and finding out that something might be wrong that you have to fix is kind of a pain, but the devastation of going through a transfer that doesn’t work is also pretty darn awful. Because I work so hard to help families, if I can do anything to prevent someone from not experiencing the devastation of a negative embryo transfer, I’m going to do everything possible to clean up that house and make sure it’s perfect.

What I recommend is that people do 3 tests before embryo transfer to make sure your uterus is absolutely perfect prior to your embryo transfer. They are: Hysteroscopy Saline Infusion, Sonogram, and Hysterosalpingogram.
