  • This episode takes a look at scans, why scans might be needed and how they can help
Scan.com was founded to make scanning accessible for clients and support earlier diagnosis making a positive difference in patient journeys and recovery
Hear about the clinical pathway for getting a scan with Scan.com and why their process supports accurate and clear information along with the next steps through an integrated system and pathways that support the patient journey

    KEY TAKEAWAYS

There is a clinical pathway that is followed alongside the option to be referred for scanning if appropriate We use each process depending on the clinical situation, but the scans are complementary to each other The patient won’t know necessarily know what the most appropriate scan will be, and when you have been screened you will be directed to the most appropriate appointment With a scan, it’s important that it’s carried out as part of a process with a clinician You can be a clinician and make a referral on the platform Ultrasounds are an important scan for peripheral muscular-skeletal conditions

    BEST MOMENTS
‘We now offer MRI, ultrasounds, X-rays and CTs’
‘Lots of reports can be so confusing for patients and we make sure the patients understand and know what’s relevant’
There’s a whole package on offer’

  • Angela Jackson, specialist paediatric muscular skeletal physiotherapist joins the podcast again to talk about all things spondy
Spondylolysis is one of the most common back pain injuries seen in child athletes and although common it's vitally important that these injuries are picked up and The Back Pain Podcast is helping to raise awareness with Angela explaining how a stress fracture is very different to a full fracture and how they can be picked up along with the actions that need to be taken

    KEY TAKEAWAYS

Lower back pain in a young athlete is an inconvenience that needs sorting out but the majority go back to sport without any problems These injuries don’t occur in the non-athlete population. Whatever level it’s at, it needs checking If you have an experienced clinician they should be able to manage it without imaging A graduated return to activities is what’s required, we want to have an adult athlete who can withstand the demands of adult sport We need to help them to see good credible role models who have come back from this injury Working in phases helps young people see what the journey is back to health It’s about getting them back into sport layer by layer, making them stronger and educating them

    BEST MOMENTS
‘It’s not that kids are doing too much, it’s that they are doing too much too soon’
‘We only get this opportunity to make you stronger once’
‘Find a hero in their sport that you know has come back from this injury, loads of people have gone through this’

  • The sacroiliac joint is an area that remains contentious despite all of the research that’s taken place and in this episode, we are chatting to Wilfried Simon, an osteopath and physio from Belgium about the misconceptions, if it can be a cause of pain and what to do if it is

    KEY TAKEAWAYS

It’s a connection point but there isn’t much movement, only 3 degrees and its deep within the spine If you want the patient to be able to treat themselves and achieve autonomy they have to understand what’s wrong When you have manipulation it’s about what it creates overall including how the person thinks about it Identify the movement that is painful and work with the patient to make the movement less sensitive through changing the axis of the movement and building the confidence of the patient to complete movements Trust the future and the process, not very day is the same, not every day is pain free but that’s what make the patient stronger and you stronger

    BEST MOMENTS

‘It’s important to treat the person not the problem’

‘There is aways a combination of what’s happening mechanically to the joint and how the patient perceives and feels about what’s happening’

  • Adam Dobson, physiotherapist, returns to the podcast to talk about the Back to Health Programme, a residential rehabilitation programme for people suffering from persistent back pain
In this episode hear not only from Adam but also from Rebecca whose experience on the programme changed her life. They discuss the challenges of being told many different things, medication and why the programme is life-changing

    KEY TAKEAWAYS

You can go to various physiotherapists and be told many different things, it can become a shopping list given to patients without any real idea of what it means to manage the problem I was pushed out of my comfort zone in many different ways but I was in an environment with people who understood and shared their experiences, it was enlightening in many ways and restored my faith in human nature In the first days of the course there will be many accounts of the shopping list of problems people have been told about their bodies When a professional tells you something you believe them because they are the expert in that area. The programme has taken on a lot of feedback and has evolved, with lots of previous patients contributing to the programme It should be a front-ended option for people with persistent pain who want to learn deeply to take control and get happy again To know with confidence that doing things isn’t going to cause any more damage has made me stronger than I’ve ever been The course gives you the confidence to understand how the body is working, what it's telling you and what’s dangerous and what isn’t

    BEST MOMENTS
‘There wasn’t one thing that was important, the whole course is important every aspect of it is important’
‘Everyone’s experiences had put them in a dark place’
‘He led us through those grotty times and showed us where we could be today’
‘It’s given me back my confidence and I do more than I’ve ever done before,
'I no longer believe the pain is causing damage’

  • Dave and Rob are having a behind the treatment room door chat about their working week as chiropractors

They discuss the importance of exercise plans and how increasing functional capacity is an important element of any treatment plan and share a great analogy to help understanding

    KEY TAKEAWAYS

Any conservative or passive care doesn’t necessarily decrease pain from issues such as osteoarthritis People assume massage is aimed at reducing pain but its about increasing functional capacity If we improve the capacity of someone’s knee health then it means they can do more Exercise increases the capacity and your threshold so you are increasing functional capacity You have to look at the bigger picture, if you knee or back is stopping you doing the things you want in life then surgery may be the option to consider If you are utilising exercise as part of your treatment plan you need to look at usage and function as well as lessening pain levels Having an expectation of no pain can result in less resilience

    BEST MOMENTS

‘You may be able to do more with the same amount of pain’

‘Pain is a very normal aspect of society’

‘We can usually increase your capacity but this may not lessen the level of pain’

  • Dave and Rob are talking about diagnostic labels and particularly those for lower back pain

Lower back pain is a complex and multifactorial condition and a specific diagnosis may not reflect the underlying causes or mechanisms for the pain so does a specific label help or hinder?

This is a learning episode where the discussion is a great opportunity to carefully consider what you do as a clinician with the evolution of diagnostic labelling

    KEY TAKEAWAYS

The research is now saying a diagnostic label is not as important as previously thought There is back pain that needs a specific diagnosis but this is not the case for the majority of non-specific back pain With non-specific back pain we know it’s nothing sinister or serious and what’s left is a combination of other things Many things can be contributing to the pain and the treatments and outcomes are similar for non-specific back pain Understanding the multifactorial and compounding aspect of non-specific back pain means there are a multitude of things you can do to help yourself The vast majority of patients will be better in 12 weeks It makes sense for us to stop using these labels and be better at explaining

    BEST MOMENTS

‘Our previous notion of a single answer to find is a bit immature’

‘A more holistic approach may lead to better outcomes’

‘This is evolving and changing our diagnostic labelling in line with current research’

  • In this episode, Rob is with Dr Sophie Newton talking about the medication both prescribed and over the counter that is used to treat back pain

What is appropriate for one person may not be the right thing for another but this is an opportunity to find out more about how different types of medication work and the reasons why specific treatments are used

    KEY TAKEAWAYS

Short-term, acute pain treatments include anti-inflammatory medication which works on damping down the inflammation Milder opioids like codeine are often given in conjunction with co-codamol Opiates work on opioid receptors that trigger the pain response Medications can be very effective but they do have side effects and it’s important that people know about these People need to become active to repair the back and it’s about the medication that can help support this Antidepressants are now recommended for chronic pain because they work on neurotransmitters The topical gels and patches do really work and for many people, it’s a much better option as the side effects profile is less

    BEST MOMENTS

‘The smallest dose is the best dose’

‘We need you to get moving and you need pain relief onboard to get moving because this is what will be the biggest help’

‘GPs are always assessing the risk and the benefits of any medication’

  • In this episode, Dave shares a detailed lower back pain case history and takes specific questions from the audience in the journey to a diagnosis

This is a fascinating insight into the approach that begins from the moment the clinician meets the patient through to the treatment journey and the factors that influence recovery

    KEY TAKEAWAYS

Whatever your discipline the tenets are the same when you talking about pain As osteopaths and chiropractors, we are talking the same words just in a different language A disc doesn’t have to present with radicular symptoms initially Back or cervical disc issues have to be considered When the range of motion is restricted isolating the areas and specific groups of muscles, joints and nerves can help with the diagnosis If the specific pain is on a full extension this is a classic sign of a set joint irritation If you experience pain over a period of time fear avoidance can become a part of the individual's experience The biggest test can be the basic test of ‘poking it’ Everything is testing and a provocative test can be seeing the patient coming into the room A test doesn’t have to have a name and make it onto your notes

    BEST MOMENTS

‘The basic tenets of good care should be what you are going by’

‘Identifying which one is the head honcho and ruling the show is what we are looking for’

‘Your provocative test starts when you greet that patient’

  • Ian Gatt is a physiotherapist with extensive experience and expertise and is an upper limb injury specialist.

In this episode Ian shares advice on tennis and golfers' elbow from the symptoms to the causes and what you can do to be able to get on with your life in the way you want to.

    KEY TAKEAWAYS

It’s something that affects a tendon and it impacts daily activities Tennis elbow tends to be on the outside and golfers the inside of the elbow It can be linked to who you are as a person and the environment you are in Having a conversation and understanding how the person feels is important and the first step You can be chasing a diagnosis but understanding the function of the person is vital so you know what they need to do and can do It’s not posture, it's technique and the repetition of a technique can cause the issue It’s about planning, taking a step back when you experience the symptoms and doing things in a different way If you are feeling something, listen to your body and give yourself a breather

    BEST MOMENTS

‘It’s important to understand the nature of the symptoms’

‘We just need to understand what the person can and can’t do with the focus on the can’

‘I use the formula of prognosis equals diagnosis plus function’

  • Dr Clodagh Dugdale Consultant in Sports and Exercise Medicine working at the Spire hospital in Cambridge discusses relative energy deficiency in sport

Learn what RED-S actually is, the signs to look out for and how it’s a syndrome that affects not only elite athletes but also recreational athletes and other cohorts

This is a must-listen for anyone from professionals to weekend warriors in understanding the symptoms of RED-S and raising awareness

    KEY TAKEAWAYS

It’s a mismatch of energy intake and energy expended, it’s a low energy availability status There is a normalisation of certain symptoms such as a lack of periods and rates of depression and anxiety are much higher in those who have RED-S Coaches and trainers are the key because they see people every week and would see the changes A lack of periods can be an early sign of RED-S and should be investigated It’s can be about both the workload and the dietary intake and a multi-disciplinary approach is the most effective way to treat RED-S People with RED-S are often post viral most of the time and low in iron Currently, it's about raising awareness about it with training and education Any treatment must be bespoke as the time and types of food are also factors in taking in enough energy There are many external factors as well as internal factors that need to be taken into account

    BEST MOMENTS

‘We need to raise awareness of all the symptoms throughout all levels of sport’

‘If someone has reduced periods they should go and see someone as its most likely a result of low energy availability’

‘There are a lot of doctors who don’t know about this’

  • In this episode, Angela Jackson specialist paediatric physio shares great content about the common conditions and injuries for children and the best treatments

It’s the art of the therapist to help educate young people on how the journey to recovery will look and what they can do to increase their level of robustness and movement competency and Angela shares great advice

    KEY TAKEAWAYS

In loads of different ways, kids aren’t adults until they are in their early twenties My first question is always ‘what’s changed’ when I’m treating children A massive part of the therapy role is helping young people to understand what contributes such as diet, sleep and exercise Girls skeletally mature around two years earlier than boys There is a finite window around when these conditions occur With the right management and knowledge you can stop the recurrence of the condition There is a role for soft tissue therapy and building the muscles up so that the child is more robust They need to develop movement and physical literacy, the confidence to move

    BEST MOMENTS

‘Growth in the foot moves to growth in the shin and then in the thigh’

‘It’s not just about the pulling and pushing it’s also about the squashing’

‘To make them more robust and resilient you need to keep them doing as much as they can to keep the muscles strong’

  • In this episode hear about the region Beta paradox -the phenomenon that people can sometimes recover more quickly from a major incident and intensive pain than from mild pain or a less distressing experience

Rob also discusses the importance of explaining to your healthcare professional why you have made an appointment and what you want to get back to doing to ensure the treatment plan is successful

    KEY TAKEAWAYS

People often mistakenly think it will take longer to recover from a major incident or intensive pain than when they have a mild pain or mild distressing experience We often only take action when the pain reaches a particular threshold When something isn’t too bad we may choose to ignore it When it’s awful we will do something about it taking action that makes it better faster As a patient it’s important to tell your practitioner why you have booked in now and what you want to get back to doing Explaining ‘why’ helps to inform the direction of the treatment plan It's more than just making the pain better its getting you back to the activities you want to do

    BEST MOMENTS

‘You may get stuck in region Beta where the less worse thing lasts longer than the worse thing’

‘If we don’t know what you want to get back to, we might not fully prepare you for the demands of it’

  • Rob and Dave are joined for a panel discussion by four other chiropractors who are all working in elite sport

They discuss the role of chiropractors in multi-disciplinary teams and the vital role they play in making sure the needs of very diverse athletes are effectively met

The discussion also covers the ways chiropractors can develop their networks and become part of the teams supporting athletes at global events, listen in and enjoy

    KEY TAKEAWAYS

Athletes have a greater understating of their bodies and are able to adapt more quickly With athletes it’s the level you’re trying to get them back to, it’s a much higher level Its impact exercises in a controlled environment leading up to full contact outcomes Its graded exposure through looking at what an individual can do Your body adapts to a specific sport You never know who you are going to meet on the journey and you will be increasing your skillset and network The best practitioners work at their craft, get mentored and make connections Specific strength training for young children could support them in high impact sport in the future COPS - chiropractor, osteopathy, physical therapy and sports massage is providing a multi-disciplinary team that can meet the diverse needs of the athletes Each profession grows as it's exposed to other professional identities We should have core skills and reach out from that point

    BEST MOMENTS

‘Athletes are invested in getting better and will do what is needed’

‘Now decision-makers want a diverse muscular skeletal workforce

‘It’s our differences and working together that makes us better

  • This week Rob is talking about the worst advice he sees given for back pain and is explaining why you definitely shouldn’t be following it

He looks at abdominal bracing, stopping sports and activities and why movement is exactly what we should be doing supported by a skilled clinician to make sure the movement is what works for you, listen in and hear more

    KEY TAKEAWAYS

You actually only need to use around 2% of your abdominal muscles for everyday activity so there is no need for extra bracing People are under the assumption that their core needs to be active all the time Move from the spine, that’s what it’s meant for Pain is multifactorial and there is rarely a single cause A skilled clinician can work out what’s the best exercise for someone at a particular time The exercise is the one that’s best for you and one you enjoy, so you will do it

    BEST MOMENTS

‘Since the nineties, the core has been blamed for just about everything’

‘We are meant to move our spines’

‘Pain is a warning sign from the body it doesn’t mean damage’

  • In today’s episode, Dave is talking gym, CrossFit and getting back into exercise in response to questions from across the Back Pain Podcast community and socials

Dave takes a look at the role of deconstructing and building towards a particular movement and why even if you have a wonky squat its ok because everyone is different

    KEY TAKEAWAYS

Full body movements are done in a group setting at high intensity Often cornerstone exercises are done and act as a marker for the level of fitness All exercises are positive if done well. It’s not the exercise that’s wrong it’s the narrative around it Humans are not all the same We are all different so we should act like it. We won't all conform to the same baseline Deconstruct the exercise into its components and work at it as long as you are building and progressing you can be ok with what you are achieving It's better to be fighting towards the next level even if you don't ever get there Human movement has a bell curve and you may be an outlier

    BEST MOMENTS

‘People who do CrossFit bloody love what they do’

‘Never stop aiming for the perfect movement but be ok with it if it never comes’

‘Some people just do have a bit of a wonky squat’
