  • On today's Egg Whisperer Show podcast episode, I'm sharing the four things you need to know about PCOS. Too many doctors tell patients that they don't need to worry about managing their PCOS until they are starting a pregnancy (not fertility docs!) This just isn't true. There are tried and true steps that you can take now to bring HOPE back to your health and your fertility.

    I want to start a PCOS revolution, and to start that, I've started calling it HOPE syndrome instead.

    H: High Testosterone (or signs and symptoms of high androgens)

    O: Ovaries with many follicles (typically over 12 per ovary)

    P: Periods that are irregular

    E: Exercise and Eating healthy habits are important to have

  • You likely already know Heidi Murkoff - she is the author of the internationally bestselling What to Expect series of pregnancy and parenting guides, which includes What to Expect When You’re Expecting. (And knowing how beloved this book is, it probably comes as no surprise that this popular book has sold over 40 million copies in 42 languages). It goes without saying that I am beyond honored to have her join me today as the guest on The Egg Whisperer Show.

That‘s roughly 3.8 Million babies born last year alone! You don’t have to be a fertility doctor to know what that means. Millions of people just like you were trying to conceive.

    Research tells us that most couples conceive within six cycles of timed intercourse, but many others (roughly 10% of couples) are struggling with fertility.

    Here’s the funny thing about what I do. I want to teach you how NOT to be my patient. Of course, there are exceptions to that statement. But in general, I’d much prefer to share with you what I’ve learned about preparing for pregnancy in hopes that it makes your journey to parenthood easier.     Secondary infertility affects so many of my patients. In today’s podcast episode, I’m talking about what happens when someone has no trouble getting pregnant for baby number one and then struggles to conceive another child. People in this situation often don’t know what to do because this is so new to them. I’m sharing everything you need to know about secondary infertility: the causes, treatments, and some of my tips.

    The truth is, secondary infertility is overwhelming and upsetting. You want to make sure that you have people around that support you, so I also encourage you to share this episode with your family and friends, so they know how to support you going through something that can be soul-crushing. I’m a very positive person, but I understand how hard it is to be a fertility patient.

  • Today on the Egg Whisperer Show, I’m excited to be joined by Dr. Nabil Arrach. Dr. Arrach is the Scientific Director at Progenesis, Inc. He has 20 years of research experience in molecular genetics, both in preclinical and clinical settings. He was the first scientist to optimize and validate next generation sequencing for PGT-A and PGT-M and he continues to work on new emerging fields in IVF.

  • In today’s Egg Whisperer Show Podcast, I’m so excited to be joined by Dr. Paula Amato. She is a fertility specialist who froze embryos in her 30s using sperm from an anonymous donor. Later, Dr. Amato decided to donate her embryos. She shared about this journey and what she learned as a fertility doctor in a recent article published in Fertility and Sterility titled: "The fertile window: what donating my embryos taught me about being a fertility doctor."

    We’re going to talk all about her decision to donate her embryos, what looking into the genetics of those embryos taught her, and what she learned from the process. It’s an amazing story, and it’s interesting to hear a fertility specialist talk about her experience as both a doctor, and a patient.

    Dr. Paula Amato is a Professor of Obstetrics and Gynecology, at the Oregon Health & Science University School of Medicine in Portland, Oregon. She also is a Reproductive Endocrinology & Infertility Specialist and has over 33 years of experience in the medical field. She graduated from University of Toronto medical school, and did her fellowship at UC San Diego.

  • Today on the Egg Whisperer Show, I’m delighted to have the inventor of the IVF Pyramid joining me!

    So often in IVF, we talk about things in terms of depletion. What are you losing? For example, you start off with a certain number of eggs at the beginning of the cycle. That’s called the antral follicle count. As the cycle progresses sometimes not all of those eggs grow. In the end, for example, you may gain two healthy embryos from twenty follicles and so it is a natural and normal human response to feel like something is lost and that something was taken away from you. I want to flip that on its head.
    Instead, you’ve started with twenty eggs from which you’ve now gained two embryos. So it took twenty follicles to get the two eggs from that cycle. Dr. Ezzati’s pyramid makes this concept much easier to understand.
    I’m excited to have this conversation during National Infertility Awareness Week! Understanding your own personalized IVF Pyramid, and, focusing on what a patient gains during IVF are two ways we all can revolutionize the world of fertility medicine. It just takes talking about the whole process a little differently to help you know more, and I can’t wait for you to hear how Dr. Ezzati explains it.

    The time to talk about "everything I can possibly do to give myself the best chance of pregnancy success" is BEFORE your embryo transfer. Not after. This is why I discuss tests like the ERA test and Receptivadx test with all my patients before transfer. Every embryo is important to me and I want to make sure I'm doing the very best job for every patient I meet.⠀⁠

    I also know that there's a virtual alphabet soup of three letter abbreviations that surround the whole fertility journey, so this week on the podcast, I'm breaking down the steps that I take when doing the ERA (Endometrial Receptivity Analysis) test, and how it could help you. The ERA tells us exactly when to start progesterone before a transfer to give the embryo the highest chance of implantation success. Think of it as a personal evite to your Embryo Transfer Party! In a way, the test tells you exactly when to show up for the party. I will also talk about the Receptivadx test that basically tells you what to wear to the party (the protocol you should use).⠀⁠

    Ever wondered what the heck AMH stands for? It’s been a hot topic recently, as women are getting their AMH levels tested. AMH is short for Anti-Müllerian Hormone – a nifty little hormone secreted by the egg-surrounding cells in your ovaries. Think of it as a fertility fortune-teller with a murky crystal ball, giving you some valuable clues into your egg stash. The funny thing about AMH is that I also like to call it the “Always Meandering Hormone,” because your AMH levels can wander month to month and year to year.

    Does a low AMH mean you're out of eggs, or going to have a difficult time getting pregnant? Not necessarily!

    And a high AMH doesn't guarantee you’ll have an easy time getting pregnant . Remember, it's not the be-all and end-all of fertility assessments.

    That’s why you might get different AMH readings depending on when you test. And, it’s also why I like to test FSH and Estradiol levels in addition to AMH to get a full picture of a patient’s fertility levels.

    Today on the blog, I’m doing a deeper dive on AMH, and what you need to know about it. I’ve had too many people question if their AMH is wrong, and what that means for IVF, and so I wanted to share all my thoughts with you.

    So, don't go scheduling weekly AMH check-ups like they're brunch dates. Remember, it's a slow and steady decline, kinda like the plot twist in your favorite rom-com. So, keep calm and check your AMH when needed – no need for monthly updates, folks!

    Have you ever used Google Maps and been redirected midway via a better route? Think about this show as your personalized navigation tool for fertility and IVF, as Dr. Aimee shares the five things you need to know before, during, and after IVF to support yourself.

    1. Be clear on why you’re considering IVF. Why are you doing IVF?

    2. Realize your relationships may change. This is something I ask my patients to think about before, during, and after IVF.

    3. Develop a plan for unused embryos. When you’re going through IVF this is likely the last thing you want to think or talk about.

    4. Know that pregnancy isn’t always easy. Developing a support system is important during IVF and fertility treatment.

    5. Understand that your feelings may linger. The feelings you had when you had a hard time getting pregnant can sometimes continue during pregnancy, after pregnancy, and beyond.
